ALL Payer PC-ACE Pro 32 EULA (End User License Agreement)
Clients MUST download, print,complete and sign the End User License Agreement BEFORE the software can be installed. M.A.S.H., Inc. will accept FAX copies of the form at our toll-free FAX number (877)350-7456 for order processing; however, we kindly request that the completed and signed forms be mailed to us with original signature in ink. Please note that orders will not be installed until we have received the original forms in the mail for countersignature by the Management of M.A.S.H., Inc. A fully executed copy will be sent back to the ordering practice.
